Please complete the form below, please note you will be asked to upload the following documents, please make sure you have them to hand:

    • Proof of Citizenship (Birth Certificate or Passport)
    • Passport-size color photograph
    • Immunization Card/Records
    • Report cards and progress reports for the previous 2 years
    • You will need a Reference Letter from the Principal or Main Teacher at the student’s current school, please download that form here.
